BHS 2019 Orientation Information

  • On Monday, August 19th, from 3pm – 5pm in the Health Professions, Nursing, Pharmacy (HPNP) Auditorium  (room 1404),  there will be a Mandatory Orientation for all students admitted to the Bachelor of Health Science program. You cannot attend the BHS Program classes unless you attend the mandatory orientation, so record the date now. During the orientation, we will review BHS policies and procedures, assist you with registration issues, address questions, and provide you with the opportunity to meet college faculty, advisors, and fellow students.

    All items in your Bachelor of Health Science Academic Record should be complete before Orientation.

    • Submission of your updated TB and Varicella (2 part vaccine) or titer test to the UF Compliance Office

       


If you are unable to attend and feel your situation qualifies as an extreme hardship, please fill out the form below. Make sure to upload any documentation you have to support your request. (Examples: Letter from doctor with dates you are under their care, receipt of travel purchased with flight schedule, letter from apartment manager). Part of the review of an absence request is to review your BHS checklist. If your checklist is not complete, you will NOT be excused.


Request to approve absence from the Bachelor of Health Science Mandatory Orientation

  • The Bachelor of Health Science Orientation is a mandatory event. In specific cases, with advance notice and proper documentation, attendance may be excused by our director. In the event you are unable to attend please complete and submit the form below no later than 10 days before the orientation. Upon review of your request and verification the supporting documents, you will be notified of the directors decision by email. If your request is granted you will be required to set up a meeting with Dr. Moorhouse (within 10 days of the start of classes) to go over what you missed at orientation. In the event your request is not granted and you do not attend the orientation you may be placed on BHS probation.
