Honors Code Verification of Academic Honesty

VERIFICATION OF ACADEMIC HONESTY AND WAIVER TO REVIEW STUDENT CONDUCT/HONOR CODE RECORD

  • Background:

    Magna and Summa Cum Laude distinctions for College of Public Health and Health Professions (PHHP) students are reserved for individuals without conduct or honor code violations based on the University of Florida Student Code of Conduct and the Student Honor Code. This policy applies to conduct and honor code violations that occur at any time at the University of Florida, from freshman through senior year. At the time of admission to the honors program as a junior, you verified that you had no conduct or honor code violations (or you were granted admission to the PHHP honors program based on an appeal of a minor infraction). Consistent with college policy, you must now verify that you have had no conduct or honor code violations during your junior and senior years in order to be eligible to graduate magna or summa cum laude (if all other honors requirements are met). Any conduct or honor code violation that occurs in the junior or senior year (after limited access admission and until the awarding of the diploma) renders a student ineligible for magna or summa cum laude graduation distinction.
  • Student Signature verifying Academic Integrity and Authorizing Waiver:

    My signature below is verification that I have no violations of the University of Florida Student Code of Conduct and the UF Student Honor Code. I give permission voluntarily for a representative of the College of Public Health and Health Professions Dean’s Office to review my University of Florida record of student conduct to determine my compliance with the College’s honor requirements necessitating no violations of the Code of Student Conduct and Student Honor Code while I have been a student at the University of Florida.
