Immunization Requirements

As part of your admission to the BHS program, you must have and provide documentation of the varicella (chicken pox) vaccination series or a positive titer (the titer test is a blood test that shows that you have had chicken pox), a TB test, and up-to-date immunizations. All immunization forms and associated documentation must be faxed or hand carried directly to the Student Health Care Center. Please read the appropriate instructions below to complete this documentation based on whether you are a UF student or a transfer student.

A. Students currently at UF:

  • Varicella (chicken pox)

If the original immunization form you turned into the Student Health Care Center (SHCC) did not include documentation of the Varicella vaccination (chicken pox), you can satisfy the Varicella vaccination or immunity requirement by providing documentation of one of the following to the Student Health Care Center. Do not send the form to the college. documentation of varicella (chicken pox) vaccination or a positive titer (i.e. immunity because you had chicken pox),

  • Other Immunizations

You must ensure that all of your immunizations are up-to-date. Your admission to the junior year in PHHP is contingent upon all required medical documents being on file with the Student Health Care Center by the deadline. Your admission will be rescinded if you do not meet the deadline. For additional information about immunizations, see the Student Health Care Center website at http://shcc.ufl.edu/.

  • Tuberculosis (TB) Skin Test

A negative TB Skin Test (or chest x-ray if the TB test is positive) taken after 5/1/15. Both Shands Student Health Care and the SHCC on Fletcher Drive administer the TB Skin Test every weekday but Thursday. If you receive a TB Skin Test through Shands Volunteer Services, you must ask for a printout to provide to the SHCC. See checklist for deadlines. The TB Skin Test is required each year of the program and must be documented on the mandatory UF Immunization Form found at http://shcc.ufl.edu/files/2011/09/ACL-002.pdf . If your TB Skin Test is administered by an outside health care facility, you must take the SHCC mandatory form with you to document the results.

B. Students Transferring to UF:

In addition to the University immunization requirements, you must submit documentation of two other immunizations to the Student Health Care Center: Varicella vaccination (chicken pox) and a Tuberculosis (TB) Skin Test.

  • Varicella

You can satisfy the Varicella vaccination or immunity requirement by providing documentation of one of the following to the Student Health Care Center. Do not send the form to the college.

i. Certification date of Varicella vaccination or a positive titer from a health care provider. You must use the following form and give it to the SHCC: http://shcc.ufl.edu/files/2011/09/ACL-002.pdf

ii. Medical documentation by a physician of the date you were treated for chicken pox. You must use the following form and give it to the SHCC: http://shcc.ufl.edu/files/2011/09/ACL-002.pdf

  • Tuberculosis (TB) Skin Test

Please retain copies for your records
